A username, which is also referred to as a logon name or user account, is a method to validate you as a legitimate user of a system. Individual usernames on systems at the College are standardized in the format first initial followed by last name. An exception to that standard occurs when more than 1 person has the same first initial and last name. In that case, the username becomes first initial, followed by a sequential number, followed by last name, e.g. J1SMITH for John Smith. If you have accounts on more than 1 system, you should have the same username for each system. User Accounts for BSC Faculty, Staff and Administrators will stay active as long the person is affiliated with BSC.
Faculty, librarians and staff requests for all new computer accounts should be initiated by printing and filling out a User Account Application Form. Please complete all sections. Sign and date the form and acknowledge your agreement to adhere to the guidelines outlined in the BSC Security Procedures. The Department Head Signature is required.
Please note that under the Please check the appropriate systems, it is acceptable to check as many accounts as you need. If you are not sure as to which systems you need, please read the Guidelines for Requesting Computer Accounts section.
Forward the completed form to:
