Media Services Equipment Request

Request Form:

To reserve equipment, please check and mark the appropriate area. Equipment must be picked up at the Westside Support Center located in the bottom floor of Maxwell Library.

BSC Connect Card is required for all pickups.

Note:  Equipment is limited and is signed out on a first come-first serve basis.
